For his follow up Frank attends the electrophysiology clinic. Patients who have had a CRT-D implanted require a clinical assessment and interrogation of the implantable cardioverter defibrillator (ICD). The nurse encourages Frank to consider his own priorities and set his own agenda. She gives him additional resources to enhance his understanding generally and also web addresses that he may want to access.
Frank leaves the clinic to talk things over with his wife.
- ICD Nurse: Hi Frank, from the test results I can see the device is working well, but how are you feeling in yourself?
- Frank: I’m not as breathless now but I still can’t do things as before
- ICD: Things will be different now but can I ask what is concerning you the most just now?
- Frank: I’m not sure I can do my job and this box reminds me I am useless and won’t be able to support my family the same as I always have
- ICD: I am sorry to hear that, but there may be other forms of employment you could consider. This must be challenging for you. Are you managing to get out of the house? Have you gone back to the darts team?
- Frank: I can’t face going out at the moment, what if I get a shock and and I don’t know what to do?
- ICD: Its important for yourself Frank to try and get back to doing normal activities that you enjoy. The device is here to support your heart and we perhaps need to revisit the information that you and your wife received after the operation. Have you spoken to your wife about your worries?
- Frank: No, I don’t want to trouble her, she is under pressure too and now that I can’t work I feel like I’m under her feet all day.
- ICD: It sounds like the work aspect of your life with the ICD is causing you the most concern, I think we should discuss options that may be available to you but also look at any benefits you might be entitled to, does that sound ok?
- Frank: Thank you for that, its so nice to be able to talk things through. Maybe I should go home to Betty and think about jobs and look at the benefits info that you have given me.
