Stroke liaison nurse from neurorehabilitation team visits Michaels home

Michael’s attention issues:

Michael does not appear to be aware or concerned about how he is managing (or not managing) at home.  During his brief stay in hospital staff told him he had made a good recovery. He has a medical certificate from his doctor to be off work for two more weeks. After this time he is expecting to go back to work, although Jane tells the stroke liaison nurse that she is not aware that her father has made any contact with his work. 

After discussing these concerns Michael agrees to an occupational therapy assessment and the nurse makes the referral to the rehabilitation team. It is also agreed that Michael will not return to work until after his occupational therapy assessment. 

Following her visit the nurse makes an onward referral to occupational therapy (with Michael’s permission) within neurorehabilitation services.  

 

  • Scene one

Jane: Dad this is the nurse that told you was coming to visit. I am a bit worried about you dad. Since going home I don’t think you have been coping. So I arranged for the nurse to come and visit. You haven’t been yourself. I think you are not managing with keeping the house tidy and you haven’t been keeping in touch with your friends.

 

  • Scene two

Jane: My dad has been a bit vague and withdrawn. Things are not right but my dad does not seem to be concerned.

Nurse Jane has contacted me too because she is a bit concerned about how you are coping since you came out of the hospital. My role is to provide people with support following discharge from the hospital after they have had a stroke. How have you been getting on since you came home, Michael?

 

  • Scene three

Michael: I don’t see what the fuss was about, they didn’t keep me in hospital for long and they said that I made a good recovery.

 

  • Scene four

Jane: Dad, I’m really worried about you. Since coming home, you haven’t been coping. The house isn’t as clean as it normally is, and you seem withdrawn and not interested by much. You haven’t been yourself.
