Q. Which of the following steps would you take next with Edith.
1. Dipstick – Yes – A dipstick gives an indication of whether there are any abnormalities in the urine and may potentially highlight a UTI.
2. Discontinue diuretic therapy – No – No not necessarily. The decision to discontinue her diuretic needs to be taken in consultation with her GP. The type of diuretic may not necessarily exacerbate her urinary symptoms and the exact nature of the ‘water pill’ needs to be investigated further.
3. Exclude UTI – Yes – Well done. This is an important factor that needs to be excluded. Urinary tract infection (UTI) may exacerbate or be totally responsible for her urinary symptoms.
4. No further assessment – irresolvable impact of stroke – No – Edith should be reassured that assessment and treatment are available to manage her symptoms and resolving the problem is achievable.
5. Insert a catheter – No – Other assessment and management options should be explored.
6. Start a fluid balance chart – Yes – If Edith completes a three day fluid balance chart this should provide a true picture of her frequency of micturition.
7. Perform a post void bladder scan – Yes – Because Edith said she often needed to return to the toilet just after finishing there this could be an indication of incomplete voiding. A post-void bladder scan would demonstrate whether or not this was an issue.
