The Multi-disciplinary team meet to start discharge planning to get Steven home to continue his recovery and rehabilitation
OT
“Steven completed his kitchen assessment and although he had some difficulty following my instructions, he managed some problem solving. We haven’t completed the cognitive screening test due to his aphasia.”
Physio
“Steven is mobilising independently and is safe on the stairs.”
Nurse
“Steven is managing to mobilise on the ward, but he needs assistance with washing and dressing. He isn’t really interacting with the other patients in the room.”
Doctor
“If Steven is safely mobile and medically stable the team should aim for discharge as soon as possible.”
SLT
“Steven has comprehension and expression difficulties but he is beginning to ask for help and use more non-verbal communication. He is orientated and his non-verbal reasoning is good and he is showing more insight into his difficulties. I will be working with his family on how to support his communication and start arranging for him to attend speech therapy from home.”
