History of event

  • Symptoms: stopped speaking, right face drooped and unable to use right arm or stand
  • Onset: 09.00 up and about, witnessed by husband,  Current time interval since last known well: 2 hrs (120 mins)

Past medical history

  • Hypertension
  • Previous angina

Relevant medications

  • Aspirin
  • Statin
  • ACE inhibitor

Relevant social history

  • Living circumstance: at home with husband
  • Prior function and care needs: independent
  • Work/hobbies: retired, bingo, reading

Examination

  • Pulse: 72, BP: 175/92
  • Neuro:
    • Handedness: right handed
    • NIHSS:  22
    • Deficits: global aphasia, severe right hemiparesis, inattention right visual field

Investigations requested with non imaging results

  • ECG SR
  • LVH
  • Recent blood tests normal
