The occupational therapist completes a cognitive screening assessment & comprehensive occupational therapy functional assessment which includes a kitchen assessment of Michael preparing scrambled eggs and toast.
Click here for Michael’s completed cognitive screening assessment (see attached PowerPoint).
Michael’s cognitive assessment findings suggest that he has difficulty with both sustained and divided attention. This was supported by the finding from the kitchen assessment during which Michael cooked the eggs but lost track of what he was doing with the toast, suggesting a difficulty with dividing his attention between the two tasks.
Michael found that his decreased attention increased the challenge for him keeping track of his daily tasks and appointments.
It is agreed that Michael will not be returning to work at this time.
