Anticipatory care planning is about thinking ahead for future changes in your health and wellbeing. Having an Anticipatory Care Plan (ACP) can help communicate your wishes when you are no longer able to make decisions. An ACP is usually developed over time with shared decision making between the person, those important to them and their care team. Creating an ACP is voluntary and can be amended at any time as people can change their mind and is not legally binding. In Scotland, information on the anticipatory care plan is recorded in the Key Information Summary (KIS)
